I agree to waive all claims and relieve Matiya World Inc., the Florida chapter and all of its Officers, Agents, Volunteers or Employees from any liability in connection with my participation in this Convention – whether it be from injury, accident, illness or death occurring during or by reason of my participating in this event. With this waiver, I also provide Matiya World Inc permission for the full use of any photographs or video during this Convention for the purpose of Marketing, Advertising or other Promotional purpose.
